Kingston, N.Y. – September 22, 2014 – Residents of New York's Westchester, Rockland, Putnam, Orange, Sullivan, Ulster and Dutchess counties are invited to take a short survey about their health and the services they use. The goal of the survey is to learn more about the health care needs of the community to identify and plan localized healthcare delivery in the future.
The survey is anonymous, will take less than 10 minutes to complete and is available online. Participants are encouraged to share the survey link and information about the survey with friends, family and other residents of the Hudson Valley.
This is being launched as part of a comprehensive Community Needs Assessment focused on local health care, an essential element of the New York state Delivery System Reform Incentive Payment program (DSRIP). The overarching goals of DSRIP are to transform the healthcare delivery system in New York, reduce avoidable hospital use, improve access to other healthcare services; thereby reducing Medicaid spending. The Community Needs Assessment, one of the first steps in the DSRIP plan, is used to identify the most pressing needs of the Hudson Valley residents and select DSRIP projects most appropriate to impact those needs.

DSRIP promotes community-level partnerships and provides financial support to healthcare providers to implement programs to better meet the needs of their communities, with a particular focus on behavioral health. This survey is being administered through a collaborative effort between HealthAlliance of Hudson Valley®, the Center for Regional Healthcare Innovation at Westchester Medical Center, Montefiore Medical Center, and Refuah Health Center Performing Provider Systems (PPS).
The survey will be made available through several Hudson Valley healthcare provider websites and offices, including HealthAlliance of the Hudson Valley®, the Institute for Family Health and the Ulster County Department of Health and Mental Health. About the Participants
The Center for Regional Healthcare Innovation at Westchester Medical Center, Montefiore Medical Center, Refuah Health Center, and HealthAlliance of Hudson Valley Performing Provider Systems (PPS), are partnerships of regional care providers who will collaborate to create integrated healthcare systems outlined in the Governor’s Delivery System Reform Incentive Payment (DSRIP) Program. Each PPS is one of the 42 ‘Emerging PPSs’ selected by New York State to participate in the DSRIP program. PPSs include practitioners from all sectors of healthcare including hospitals, physician-based practices, nursing homes, Federally Qualified Health Centers, behavioral health providers, health homes, and community-based organizations.
